Volunteer Back Ground Check Information & Form

 

 

There has been a change in the approach to volunteering throughout the Archdiocese of Hartford. This will affect St. Thomas the Apostle School and the activities we organize for our school and our students.

 

All volunteers who work with or around students will be required to complete a background check form. The form will be submitted to the Chancery for processing. The goal is to make sure that all people working with our children are free of any previous issues that would jeopardize their safety.

 

Any field trip chaperones, teachers’ helpers, activity coordinators that involve spending time with students, will need to complete the form. There is a fee of $20.00 for the filing and processing.

 

Because this charge was not built into our school budget for this year, we are asking for each parent who wishes to participate in student-centered activities to donate the $20.00 for the processing when they submit the form. This is a one time charge. You will not be required to complete or resubmit the form again in your time at STA. We can accept cash or check made out to St. Thomas the Apostle

School.

 

The forms will be available in the office. Teachers will also be able to send home forms for completion when and if you are interested in field trips or other student- centered activities.
